Use point scale:
0 = Never, 1 = Rarely, 2 = Sometimes, 3 = Often, 4 = Very Often
I find it difficult finishing a project once the challenge is done. (note: If it happens "often", you put 3 point )
I find it difficult organizing a task in order.
I have problems remembering meetings or appointments.
I avoid or delay getting started if I have a project or task that requires a lot of thought (sustained thinking).
I find it difficult sitting down for a long time and often I fidget or squirm with my fingers or feet.
I find myself feeling "overly active" or acting as if "driven by a motor".
Results:
If you have four or more of the following...
2 points or higher on above questions 1, 2, 3
3 points or higher on above questions 4, 5, 6
You have "symptoms highly consistent with ADHD in adults".
